Acknowledgment of Risk:
I, the undersigned, understand and acknowledge that participation in dance classes, rehearsals, performances, and related activities at Elektro Dance Academy involves inherent risks, including but not limited to physical injury, strain, and accidents. I voluntarily assume all such risks associated with these activities.
Release of Liability:
In consideration of being permitted to participate in dance classes and activities at Elektro Dance Academy, I hereby release, waive, and discharge Elektro Dance Academy, its owners, instructors, employees, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, loss, or damage that may be sustained by me while participating in any activity, whether caused by negligence or otherwise.
Medical Treatment:
I grant permission for Elektro Dance Academy to obtain emergency medical treatment for me in the event of an injury or illness during my participation in activities at the studio. I understand that I am responsible for any medical expenses incurred.
Photographic Release:
I consent to the use of my image, likeness, and/or voice in photographs, videos, and other media by Elektro Dance Academy for promotional and marketing purposes without compensation.
30 day drop policy:
I understand Elektro Dance Academy has a "30 day drop" policy. If I intend to quit the academy I must provide 30 day notice. Failure to provide this notice will result in being charged one month of tuition as a penalty.
TUITION POLICIES:
Tuition is due on the 1st of the month. All families are required to have a credit card on file and tuition will automatically be withdrawn each month. Families are welcome to use a different card, cash or check before the first of the month to pay tuition. Accounts with balances 30 days overdue will be placed on hold and dancers will not be permitted to participate in class until the overdue balance has been paid. If an account reaches 45 days overdue the student will be dropped from the class. Accounts reaching 90 days overdue will be sent to collections. All monthly tuition fees are discounted rates. It is the same regardless of whether there are 3,4, or 5 weeks in a month. If you don't pay monthly, regular rate per class is $20 per hour. No refunds are given for tuition, recital fees, costume fees, performance fees or competition fees. Returned check fee is $25 plus the late fee. Payments are applied to oldest balance first.
Agreement to Terms:
I have read and fully understand this Liability Waiver & Release Form as well as the studio policies. I acknowledge that by signing below, I am giving up substantial rights, including the right to sue. I sign this waiver voluntarily and without any inducement.
